Community Response Service


What is the Community Response Service?

The Community Response Service is aimed at helping you to regain your independence so that you can carry on living at home for as long as possible and supports adults over the age of 18 with physical disabilities, mental health problems and older persons.

Who can get help?

  • First you will need to be assessed to see if you meet our eligibility criteria.

  • Then we will assess your needs to see how we can help you regain your independence.

What help can the service provide?

The Community Response team offers a range of services that are available to you. An individual assessment will identify what services you need. The services available include the following:

  • Promoting Independence: we will help you become more independent in activities such as personal care and daily living tasks

  • Personal Care: this could include assisting you with maintaining personal hygiene and skin care
    Health: we will help with health related tasks such as handling of medication
    Well-being: we can assist with maintaining mobility, and social interaction and daily living tasks
    Food and Nutrition: this could include assisting with meal preparation

What happens after six weeks?

There will be a final review and assessment, either before or at 28 days, where we will decide if you still need support:

If you have been assessed as regaining your independence your Care Manager or Team Leader will close the service.

If you have been assessed as requiring long term support, your Team Leader/Care Manager will discuss your identified needs with you and arrange for an independent agency to provide you with a care package. The Community Response Team will support you during the transition from our service to the independent service provider.

How does the service work?

The Community Response team will work with you for a maximum of six weeks, although this may need to be for longer if you have specific needs. Longer term care packages will be provided by an independent agency.

The team will work with you to deliver care which is tailored to suit your individual needs. The team also aims to provide support and social re-enablement where needed, to help you with your health, hygiene, safety, mobility and emotional well-being.

The teams

Each office has a trained CSCI registered manager, supported by Team Leaders, and Community Response Assistants.

All staff have foundation training on moving and handling, personal care skills, principles and values of care, food hygiene, promoting independence, dementia awareness, emergency aid, and infection control before they are allowed to work alone with a service user.

 

Operational hours

From 7am - 10pm 365 days a year.
