For your current social worker

If you have a social worker or care package with Hampshire County Council then you should try to make contact with the named individual or team looking after your case.

You should be able to find the relevant telephone number on correspondence that Hampshire County Council Adult Services will have sent you.

If you still cannot trace a telephone number then contact Adult Services:

Please note. Contact names and numbers will not be given out over the phone. You will be put in contact with your social worker or your social worker will contact you.

For a hospital social worker

If you are still in hospital speak to your nurse. Depending on your needs and what support you have, the nurse may then ask a member of Adult Services staff to carry out an assessment of your care needs to see if you are eligible for help from Adult Services

Or you could search the NHS directory to find your local patient advice services which provide confidential advice and support to patients, families and carers who have worries or concerns about healthcare.

For people who have not been allocated a social worker

If you do not have an allocated social worker or social work team please contact Adult Services to ask for an assessment of your care needs and to find out if you are eligible for help.
