Care management practice manual
The Recruitment and Training chapter (Chapter 8) has been updated and now includes updated Progression Criteria for unqualified care managers, as well as information about induction and training for care managers in their first year of employment.
A new chapter 10 has been added which includes practice and recording guidance for Carer Assessments and Services.
The Care Management Practice Manual will remain in place as practice guidance for care management until the full review of processes for Self Directed Support is completed. A new practice manual for Self-directed Support will be published during 2010. A full review of the manual has therefore not been completed, and some sections have not been updated since 2005. Any queries relating to practice should be directed to the Professional Adviser, Care Management.
The Importance of Care Management
Care management underpins the work of the Department, and this manual helps practitioners and managers 'get it right'. The manual combines the best social work practice with a care management approach, making sure that the users of our services and their carers are at the centre of our work. These principles remain at the core of our practice:
- Each individual accessing our services will be assessed through the care management process, and their needs will be met in a sensitive and enabling way.
- To achieve the best outcomes for users of our services, the care manager will be managed independently from our service providers.
- Practitioners will be skilled, knowledgeable and open in their assessment of individual needs.
