Wellbeing & Partnerships

The aim of the team is to provide services to a wide range of people who need help and assistance to live full and independent lives.

Some of these services are developed by the team and others are run by organisations on our behalf. The team is committed to making sure that all services are of a high standard and meet the needs of those people who need and use them.

  • Drug and alcohol action - information for people with drug and alcohol problems. Links to the DAAT website for stakeholders, partners, providers and other professionals.

  • Extra care housing - or very sheltered housing offers an alternative to residential care, providing a range of quality services
  • Home Improvement Agency (HIA) - staff can help with your application for the Disabled Facilities Grant and if you do not qualify can suggest other means of funding
  • Shared lives (Adult Placement) - operates across the county and is run by Adult Services staff. Shared Lives carers take vulnerable people into their home and give them the support they need, in a family environment
  • Shared ownership - The Locality Housing Officers (LHO’s) refer clients to a particular Housing Association that will be able to purchase a property off the open market for an individual and sell them a share so they will become shared owners. The part that the Housing Association own will be charged rent upon and housing benefit can pay this if the individual is eligible
  • Sheltered housing - is mainly for older people (over 60) and usually takes the form of a group of bungalows or flats supervised by a Scheme Manager, who can give you some help and support in an emergency. Some properties are also adapted for disabled use
  • Supported housing - ‘group living’ schemes which offer communal accommodation to small groups of people in a similar situation – a kind of house-share. These schemes are mainly for people with learning disabilities and for people with mental health problems. Group living schemes are run largely by voluntary organisations or housing associations.   • Supporting People - offers a range of housing related support services and aim to offer vulnerable people the opportunity to improve their quality of life by providing a stable environment which enables greater independence
  • Telecare - technology including a range of alarms and sensors that can enable people to live safely and independently in their own homes
