Alzheimer’s Society (Hampshire Branch) – local branch of leading care and research charity.
Carers Together – consortium of carers in Hampshire.
Graphnet Health Solutions – suppliers of leading shared care record system .
Hampshire County Council (HCC) – the local authority with social services responsibilities in Hampshire.
Hampshire Partnership NHS Foundation Trust (HPFT) – trust providing mental health and learning disability services across Hampshire.
Northgate Information Systems – supplier of leading social care record system.
Portsmouth City Council - the local authority with responsibility for social care in Portsmouth
Southampton City Council - the local authority with responsibility for social care in Southampton
