    This form can be completed online and then saved, allowing you to either

    • keep a copy on your computer for your own records
    • print and then post with additional documentation and the appropriate fee to the team
    • email your application and additional documentation to the team

    If emailed, remember that a £10 fee is applicable which will need to be sent separately, by cheque, to the Information and Complaints Team.
