Adult placement

The Hampshire Adult Placement Scheme (sometimes known as Shared Care) is all about linking people who need help with people who can provide it.

Our adult placement carers take people with learning disabilities, for example, into their home and give them the support they need in a family environment. Most people who use the scheme have learning difficulties, but some have physical disabilities or mental health problems, and increasingly the scheme is providing support for older people.

What carers offer varies from person to person. For example, some carers help one person once a week for a few hours during the day, others have someone live with them as an extra member of the family.

If you are interested in becoming an Adult Placement carer, you can complete the initial application form online. You may first wish to read the information for prospective carers.

The application and assessment process if fairly lengthy and involves the completion of a number of documents which make up the Carer Application Pack. Most of these documents will be provided upon receipt of your initial application form.

Further guidance notes on the application pack (including this form) are also available online.
