Transition to adulthood

Transition to adulthood is a time when young people and their families are thinking about their aspirations for the future.

If children are likely to have eligible care and support needs, when they are 18, early joint planning will be needed to maximise the gains that can be achieved before and after transition from Children’s Services to Adult Services.

Hampshire’s Local Offer webpages provide information on the transition process and also details how the Transition Team at Hampshire County Council can offer support to young people going through transition.
