Going into and leaving hospital

Coming home from hospital

Leaving hospital and coming home can be a time of mixed feelings. Although you will probably be looking forward to getting home, you may also have some worries about how you will cope.

You will not be sent home from hospital until you are medically fit to go, but you may still need some care when you get there.

While you are in hospital a nurse will speak to you about preparing to leave hospital and your situation at home. Depending on your needs and what support you have, the nurse may then ask a member of Adult Services staff to carry out an assessment, with your consent, of your care needs and see if you are eligible for help from Adult Services.

Our web pages can also help you find transport advice for getting home.
