What is self-directed support?
Self-directed support is one of the ways in which social care is developing to support personalisation. It gives more choice and control to people who are eligible to receive social care services. There are seven steps to self- directed support:
Step 1 – Completing a supported self-assessment of your needs and finding out how much money it will cost for these needs to be met.
Step 2 – Making a plan, with the help of your care manager, family or friends, setting out how you want the money to be spent to meet your needs.
Step 3 – Getting the plan agreed by Hampshire County Council.
Step 4 – Organising the money- you can choose to have this done on your behalf.
Step 5 – Organising the support - you can choose to have this done on your behalf.
Step 6 – Living your life - with the support in place.
Step 7 – Seeing how it is working and making changes if necessary
