These guidelines emphasise the importance of the thought processes that precede the written version of an intervention plan. However, the plan does need to be recorded, so that it can be shared with other interested parties, and so that there is a clear document against which progress can be measured.
One possible format for recording that intervention plan is included. This does not imply that there is only one way of recording the relevant thought processes - simply that the plan overleaf is linked to the areas which should be considered when designing an intervention. Schools may choose to use the plan overleaf, or design their own, to reflect their individual circumstances.
