Parents who need to make an application based on medical, physical or psychological grounds.
If your child or a member of your family living at the same address has a serious medical, physical or psychological need that makes it essential that the child attends the preferred school, then you must tick the appropriate box on your child's application form to indicate this. Please bear in mind that all schools can cope with special educational needs and common childhood complaints such as asthma.
Supporting evidence must be submitted by the deadline, for example, a letter from a registered health professional such as a doctor, setting out the reasons why the school in question is essential. Your application will not be considered under this criterion if you do not provide the supporting evidence.
Applications on medical, physical or psychological grounds will be considered by the school and a panel of senior officers at Hampshire County Council. All information submitted will be regarded as confidential. If either party (the preferred school or the panel of officers) does not consider admission to the school to be essential, then the application will be considered against the remaining admission criteria e.g. living within the catchment area.
If you are applying to a church school and wish to make an application under the church criteria of the school's admission policy.
You must complete a Supplementary Information Form which asks for declaration and verification of your Christian commitment.
You must ask the school for their Supplementary Information Form, and complete and return it to the school before the deadline.
If you are applying to more than one church school you must complete a separate Supplementary Information Form for each school.
The church criterion cannot be verified if you do not submit a completed valid Supplementary Information Form and your child may not be offered a place as a result of this missing information.
