Public Health District and Wellbeing grant application notes

The Application form is a standardised layout which is used for all grants given by Hampshire County Council.  

Only Health and Wellbeing District Forum members may apply to this grant stream

To help you in completing the form, please note the following:

Organisation details (Questions 1-6)

Question 1 Department

For Department select “Public Health”

For Grant Programme select “Public Health District and Wellbeing Grants”

Select the grant for over £1,000 – standard form

Question 2 Contact details of your organisation

Organisational name – please give the name of the Forum and contact address.

Question 6 Aims and objectives

Please state “To improve health & wellbeing of people of Hampshire.” (This is a mandatory question but as applicants can only be District Health and Wellbeing Groups you can keep this very brief).

Your project/activity (Questions 7-10)

Please complete all questions to tell us about the project / activity / service you are requesting funding for and how it meets the grant criteria.  

Finance (Questions 11-15)

This question relates to finance for your project.

Question 11 Financial reserves

It is likely the District Forum has no budget or free reserves. If this is the case please select “no” otherwise enter details as applicable.

Question 12 Costs required to carry out the project/activity

Provide a breakdown of all costs of activity/project.

Question 13 Income

This is a mandatory question. The columns: ‘Name of Organisation’, ‘Confirmed?’ and ‘Amount’, all need to have one row completed. (e.g. If you do not have any other funding you will need to select own funds committed – confirmed - yes – amount “0” ).

General Information (Questions 16-20)

Question 20 Equalities

Please answer all three parts as best you can. We are not looking for comprehensive policies – just some bullet points, for example: thorough training; information sessions, staff guidebook etc. In addition to information about your policies and procedures please outline how the aims of your project will contribute towards a, b, and c.

Examples of things which may be relevant to include are:

  • We have an equality and diversity policy. We ensure our staff receive equalities and diversity training. We actively encourage and support all sectors of the community including those having protected characteristics to take part in this project.

  • We have an open and transparent recruitment and promotion policy.

  • The organisation’s culture fully embraces diversity.
