Adult Services: help us to shape the way we provide information
We want to make sure that you can find useful up to date information to help you to make choices about health and care. We want to make sure that you can give feedback about what you think of the services and care you get so that those services can be improved.
We have put together a survey to help us gather the views of service users, their carers and families about their experience of looking for information about health or social care.
We want to make sure that everyone coming into care services has all the information they and their family need in order to support them with their choices. Therefore your experience is invaluable to us. We would also like to know what information you think should be on our website and how we could begin to use it to receive comments on our services.
It doesn't matter to us whether you use the internet regularly (or at all) - the important thing is that you tell us what information is important to people who may need some care and support, and how easy it is to find the information you need.
The survey will take approximately 10 minutes to complete, we will be gathering all responses on Monday 5 August.
If you would rather have a paper copy sent to you please contact Katrina Hillman either by email; Katrina.firstname.lastname@example.org or by phone; 0845 603 5630.
