Introduction to Care Management e-Learning
Hampshire Learning Centre, working with specialist staff from Adult Services, have developed a 30 minute e-learning course to introduce you to the Care Management process.
What will the Introduction to Care Management e-Learning cover?
This e-Learning module will introduce you to the care management process and how this is applied in Hampshire Adult Services. This module looks at the way that the role of a care manager has developed, the process and changes which have been undertaken, and how the role may change in the future. The e-learning also explores how a care manager works, and the tasks they undertake.
Leaning Aim:
To provide an introduction to the care management process and how this process is applied in Hampshire Adult Services
Learning Outcomes:
By the end of this e-learning module, you will be able to:
Explain how the care management process has developed since 1988.
Describe the purpose of the seven stages of care management
List the key policies and procedures that support the care management process in Hampshire Adult Services
Discuss current developments in the care management process and be aware of the potential changes in the care manager role in the future.

About this e-Learning
Visits and progression through the course are tracked so that training via e-learning can be measured for effectiveness and provide a log as part of individual performance development
This e-Learning is aimed at any staff who wish to improve their knowledge of the care management process, but must be completed by all new care managers within two weeks of their appointment.
This e-Learning will take approximately 30 minutes to complete.
