To give an accurate picture of:

• how often the service is used

• the quality of our service

• the total cost to us; and

• the impact on customers

we need to monitor the demand for alternative formats and translation services.

Please record your use of alternative formats on the on-line monitoring form. The Equalities Team will produce an annual report based on the information they collect throughout the year.
