The Community Response Team provides short term assessment and reablement to help you regain your independence and can carry on living at home for as long as possible. It supports adults over the age of 18 with physical disabilities, mental health problems and older persons.
The Community Response Team will work with you for a maximum of six weeks. Longer term care packages will be provided by an independent agency if this is identified as a need following a re assessment by a care manager
The team will work with you to deliver care which is tailored to suit your individual needs. The team also aims to provide support and social re-enablement where needed, to help you with your health, hygiene, safety, mobility and emotional well-being.

There will be a final review and assessment, either before or at 28 days, where we will decide if you still need support:
If you have been assessed as regaining your independence your Care Manager or Team Leader will close the service.
If you have been assessed as requiring long term support, your Team Leader/Care Manager will discuss your identified needs with you and arrange for an independent agency to provide you with a care package. The Community Response Team will support you during the transition from our service to the independent service provider.
Each office has a trained Care Quality Commission registered manager, supported by Team Leaders, and Community Response Assistants.
All staff have foundation training on moving and handling, personal care skills, principles and values of care, food hygiene, promoting independence, dementia awareness, emergency aid, and infection control before they are allowed to work alone with a service user.
