Make a Subject Access Requests

As part of the Data Protection Act you have the right to request to view the personal information that a Data Controller has collected and holds on you. A Data Controller can be Hampshire County Council, your bank, your doctor, utility companies etc.

Childrens Services and Adult Services

If you want to see the personal information that has been collected and held on you by Children's Services or Adult Services then please complete the CR11 Subject Access Request Form. You will need to download and print this form and send it to the Subject Access Request Team with the appropriate payment and documentation.

If you want to see the personal information that has been collected and held on you by other departments within Hampshire County Council then please download, print and complete the Subject Access Request Form 38kb. Once completed you will need to send it with the appropriate fee and documentation to:

Please note that there are times when information cannot be released to you. This is because the Data Protection Act has within it exemptions that need to be applied where we are not permitted to release your information.

Closed Circuit TV

Under the Data Protection Act you have the right to request any recordings from CCTV owned by Hampshire County Council of you. If you want to see a recording then please download, print, complete the CCTV Subject Access Request 35kb form. Please send the completed form with appropriate payment and documentation to the Information Compliance Team at the address above.
