This form is only to be used if you are currently contributing to the Hampshire Pension Fund

Use this form if you are involved in proceedings for divorce or dissolution of a civil partnership, judicial separation or nullity. Also to be used if you are asked to get a transfer value for an independent financial advisor.

If you have a deferred (frozen) pension please send your request in writing stating the reason that you need this so that we are able to provide you with the correct quote.

This form should be completed if you have previously opted out of the pension scheme and now wish to start paying contributions again in the same employment.
Please complete part one and send the form onto your payroll provider.

Use this form to nominate someone to receive the death grant . This grant will be paid to your estate if you do not make a nomination.

Use this form to nominate a co-habiting partner to receive dependant's benefits. You will need to send evidence of financial interdependency / dependency for at least two years prior to the date of the nomination. This form is only valid for use from 1 April 2008.
