On-line Booking Request for Appointment to Register a Birth

Hampshire Registration Service now offers the above service for the following offices:

Aldershot                       Alton Andover

Basingstoke                    Fareham Petersfield

Ringwood                               Romsey Winchester

This form cannot be used to schedule next day appointments, if you require this service please ring your local Registration Office directly.

This form can be used if your baby was born anywhere in England and Wales within the last 42 days (otherwise contact your local Registration Office direct).

If your baby was born at home, we suggest that you contact your local Register Office directly to ensure the birth is registered in the right district.

You can choose to register the birth of your baby in a District other than that where the baby was born if this is more convenient for you. This is called registering by Declaration – however please be aware that if you choose this option there will be a delay in receiving the relevant documents, as these will need to be sent to you by post.

You will be contacted on the next working day after receipt of your request to confirm your appointment date and time.
