Ordering Copies of Birth Certificates
Please read these notes before filling in the application form.
Are you applying for a copy Birth Certificate?
Do you want a Standard Certificate or a Short Certificate?
A Standard Certificate is a full copy of the birth entry and includes particulars of parentage and registration.
A Short Certificate shows only the name and surname, sex, date of birth and district of birth.
When you apply for a certificate please state the full name and surname at birth of the person whose certificate is required, plus the date and place of birth (town or village and county). To help our search please provide if possible the names and surnames of parents, including the mother's maiden name.
Records of birth are kept in the registration district where the person was born (this might not be where the parents lived, or where they went to register the birth). Please send your application to the Registration Office nearest to the place where the birth occurred. If necessary it will be forwarded to the correct office.
NB: From the 4th May 2004 if you are applying for your first passport you will require a Standard Certificate.
Adoption
You cannot use this form to apply for an Adoption Certificate. Records of adoptions in England and Wales (since 1 January 1927) are held by the Registrar General in Southport.

Ordering Birth Certificates:
The online ordering system is currently under development, and will be out of commission until the end of September 2008.

You can order certificates over the telephone by contacting the applicable registration office.
