Ordering Copies of Death Certificates
Please read these notes before filling in the application form
Are you applying for a copy Death Certificate?
When you apply for a certificate please state the full name of the deceased at the time of his/her death and whether he/she had been known by any other name; also the date and place of death.
Records of death are kept in the registration district where the person died. Please send your application to the Registration Office nearest to the place where the death occurred. If necessary it will be forwarded to the correct office.
Ordering Death Certificates:
The online ordering system is currently under development, and will be out of commission until the end of August 2008.
We apologise for the inconvenience, but you are still welcome to continue to order certificates over the telephone or by post.
You can order certificates over the telephone by contacting the applicable registration office.
By Cheque - for registrations in Hampshire only (excluding Southampton and Portsmouth):
