On-line Booking Request for Appointment to Register a Death

Hampshire Registration Service now offers the above service for the following offices:

Please read these notes before completing the On-Line Booking Request.

  • You can only book your appointment if you have the Medical Cause of Death Certificate from the doctor.

  • If the Coroner has been involved in any way you will not be able to book an appointment online, you will have to contact the register office directly.

  • You cannot use this service to book an appointment the next day.

  • A death should be registered within 5 working days.

  • You can only book on-line if the death has occurred within 5 working days. (If the death occurred over any public holidays you may not be able to book online. If this is the case please telephone the relevant registration office.)

  • A death is registered in the district where it occurred. If you are unable to register in the district where the death occurred, or if it is more convenient for you to register in another district, you can register 'by declaration' in any register office in England and Wales. (See below)

Who can register the death?

  • A relative of the deceased, present at the death

  • A relative of the deceased

  • A person persent at the death

  • The occupier of the house or establishment where the death took place if they were aware of the death. (The 'occupier' would be the chief resident officer of an institution, eg the matron of a nursing home.)

  • The person making the funeral arrangements, ie the person making the decisions about the funeral.

Registering a Death by Declaration

  • You can book an appointment on-line to register a death by declaration only if the Medical Cause of Death has been issued by the doctor and only if it can be faxed to the registrar who will take your declaration.

  • If you choose to do this please note that there will be a delay in your receiving any certificates and other documents issued after registration as these will be sent to you in the post.

  • The registrar who takes your declaration will post it to the registrar of the district where the death took place where it will registered.

You will be contacted on the next working day after receipt of your request to confirm your appointment date and time.
