Saving time and sharing information

Wouldn’t it be great if you only had to give information about yourself once? Usually the first thing you have to do when using any service is fill in a form. But how often have you done this or talked to people from different organisations, then had to repeat everything?

The Say it Once website includes helpful information and forms including a Personal Profile. This form allows you to record important information about yourself and keep it in a safe place in case professionals need quick access to it.

One example could be giving the form to paramedics in an emergency so they can see vital allergy information. The information can also include things like medication, contact details for friends and neighbours and your personal beliefs. You can give as much or as little information as you wish and it’s your choice if you allow other people to share it.

There are also personal care plans for long term conditions so you can keep yourself well for as long as possible.

Guidance for care professionals

Many people are now using forms which can be downloaded from the website in different formats. These include a Personal Profile which can contain key information such as their current medication and who to contact in an emergency. Information is only shared with an individual’s agreement.

If an individual agrees to share their Personal Profile, this is stored on the Hampshire Health Record, an electronic store of summary information from GPs, hospitals, community care teams and others.

There are many benefits of sharing information. These include:

  • giving people a better experience of health and social care services
