Applying to a church school on faith grounds

For each church school you apply to, you must complete a Supplementary Information Form which asks for declaration and verification of your Christian commitment.

You must ask the school for their Supplementary Information Form, and complete and return it to the school before the relevant deadline.

If you do not submit a completed valid Supplementary Information Form, your application cannot be considered under the faith criterion and your child may not be offered a place as a result of this missing information.
