Nomination Forms

All of the nomination forms can be found below. For each category you are asked to give a supporting statement in no more than 250 words except Service to Sport and Club of the Year where the word limit is 350. This allows us to asses the relative merits of each nomination.

Within the word allowance you should explain clearly how the nominee meets the criteria for the award category.

All nominations must be supported by one of the following:

• Local Authority Sports Development Officer
• National Governing Body Officer
• Local Sports Council
