Standard 3 working with people
Standard 3 is about understanding the role of the manager in leading and developing teams and individual performance: supervisions, recruitment and induction of new staff, ensuring others’ learning, and effective communication.

Key to communication in social care is the care plan. PaCT have developed a person centred care plan toolkit. The toolkit contains generic person centred care plan templates followed by guidance on their use and the principles behind them.
When using these templates it should be noted that:
- This is not a template that HCC advise all providers to use. It is a toolkit of person centred templates that are designed as a starting point for organisations to think about developing their own documentation.
- These are generic templates designed to cover aspects of care provided by most sectors of social care. It is not specific to only one (such as residential) and may not be appropriate for all organisations.
- Each organisation using the toolkit should evaluate each section in line with their current organisational needs and adapt this to best suit their service and culture.
We have also developed some condition specific risk assessments to be used in care plans that have highlighted a specific increase in risk due to an individuals condition or change in circumstances. These are based on existing templates and have been developed to suit social care settings. Please refer to the guidance documents 44kb before using these which outlines when it might be approriate to use each one and where they were originally sourced from.

For people with learning disabilities there is a scheme called health passport. Users can experience a great deal of frustration when transfering between services. Having information available that can promote consistency of care by communicating what is important to individuals, what their likes and dislikes are and how best to support them. The health passport is a document that can provide a great deal of information to other professionals quickly.and effectively. There is also a scheme relating to end of life called Say it Once. This is an advance care plan for anyone at any age and can be held by individuals or stored on a central computer. It is aimed at informing people of an individuals wishes and could be used for person centred planning.

PaCT offer managers development days. These are a good opportunity for managers and senior staff to keep updated with latest trends and developments in social care and consists of seminars work shops and talks by leading professionals in the field.

Standard 5 achieving outcomes
Standard 5 is about delivering a quality service that is customer-forced and the manager’s responsibilities for performance management processes, partnership working, information sharing, record keeping and change.
